Sends to CRBs if unverified sales are found within the time period for specified CRBs
The Unverified Sales Questionnaire allows you to request fixes and explanations from retail CRBs when sales in their POS data appear inconsistent with compliant activity.
This article explains how to set it up, send it, and review results.
What the Questionnaire Does

Sends automatically to CRBs based on the distribution list if unverified sales are found within the time period (you can add a minimum threshold as well).

Provides a CSV snapshot of unverified sales for a specific time period.

Prompts the CRB to indicate what action they took to resolve discrepancies.
- Automatically kicks off a re-ingestion IF POS source data was changed and that option was selected within the questionnaire.

Submits their response back to you for review in Green Check.
Distribution Types
The questionnaire can be cloned and configured for specific time periods, such as:

Weekly

Monthly

Quarterly
- Semi-Annually

Annually

Or another custom frequency
When distributed on a schedule, it automatically pulls unverified sales from the selected time period and applies the threshold you’ve defined. If the percentage of unverified sales is below the threshold, the questionnaire will be suppressed and not sent.
Setting Up the Questionnaire

Clone the Master Template

Go to Settings > Questionnaires > New Questionnaire > Use a Template and locate the Unverified Sales Questionnaire (Retail).

Clone it, rename it, give it a description, and set your desired frequency (monthly, quarterly, etc.).

Set the Unverified Sales Threshold (%)
You can define a percentage-based threshold to determine when the questionnaire is sent. This prevents CRBs from being asked about small discrepancies that fall within your compliance tolerance.Example: If the threshold is set at 5%, and a CRB has $10,000 in total sales, the questionnaire will only be sent if more than $500 in sales are unverified.
This threshold applies to both scheduled distributions and one-time sends, ensuring consistent logic across all delivery methods. Click Create Questionnaire once you've added the threshold.

Turn on Distribution and add CRBs to the Distribution List

Important: In this initial version, you must add the CRBs you want to monitor for the chosen time period.

The system will only send questionnaires to CRBs on the distribution list.

Confirm Auto-Distribution by clicking Distribute Questionnaire from the modal

Once set up, the questionnaire will automatically go out on your chosen schedule to the CRBs on your list.

If a CRB has no unverified sales for the period, the questionnaire will be suppressed (not sent).

Reminder: for this version of the Unverified Sales Questionnaire, you must select CRBs to be added to the distribution list. Please remember this for newly approved accounts as this questionnaire will only auto-distribute if a CRB is on the distribution list.
Send a One-Time Questionnaire (optional)
To send the questionnaire for a specific period:

Go to Accounts > Questionnaires

Click Send Questionnaire Now

Select the Unverified Sales Questionnaire

Enter your date range

Enter the Unverified Sales Threshold (%)

Click Send
Just like with scheduled distributions, the questionnaire will only send if the percentage of unverified sales meets or exceeds the threshold you’ve entered.
Example: If you set the threshold at 3% and the CRB has $50,000 in total sales, at least $1,500 must be unverified for the questionnaire to be sent.
If the threshold is not met, the questionnaire will be suppressed, ensuring your CRBs only receive requests when meaningful discrepancies exist.
What CRBs See

They'll receive a notification that they have a new questionnaire to complete.

It will include a CSV download of the unverified sales for the time period.

They'll be required to take action and indicate what they've done to correct the issues (e.g., updated POS, corrected SOPs, contacted provider, no action, other).

If they made corrections to their POS, Green Check will automatically reingest the sales for the time period. This will happen automatically if and when the POS option is chosen and the CRB completes the questionnaire.
Where to View Responses

Accounts > Questionnaires — View the questionnaire responses associated with each CRB.

Tasks Table — See system-generated tasks linked to the questionnaire; tasks will be auto-completed once the CRB submits their response.
Tips for Use

Be selective: Only add CRBs you actively want to monitor for the chosen time period.

Review responses: Use both the Questionnaires and Tasks tables to track and follow up as needed.

Leverage the CSV snapshots: They serve as a “before” record, making it easy to compare against updated POS data after reprocessing.
