Send questionnaires back for revision
If a business submits a questionnaire with incomplete or incorrect responses, you can send it back for revision instead of sending a brand new one. The business is notified automatically, makes their corrections in the same questionnaire, and resubmits. The revision task closes on its own once they're done.
Revision is available for all questionnaires created by you and your team (as well as Beneficial Ownership). For other Green Check Managed Templates, use Send New Version instead.
How to Request a Revision
Step 1: Find the completed questionnaire
You can start from two places:
- Accounts: Go to Accounts > [Account name] > Questionnaires tab. Find the questionnaire with a Completed status.
- Tasks: Go to Tasks > External tab. Click the questionnaire task row to open the task drawer.
Step 2: Click Request Revision
- From the questionnaire results page: click Request Revision in the header action bar.
If the button is grayed out, hover over it to see why. Common reasons:
- The questionnaire is a GCV Managed Template (use Send New Version instead)
- A revision is already in progress for this questionnaire
- The questionnaire isn't in Completed status
Step 3: Add your revision details
A modal will appear with two fields:
- What needs to be fixed? — Describe what the business needs to correct or update. This note is sent directly to the business. (Required)
- Revision Due Date — Set a deadline for the business to resubmit. (Required)
Step 4: Send the revision request
Click Send Revision Request. The business receives an email with your note, the due date, and a direct link back to the questionnaire.

When the Business Resubmits
Once the business updates and resubmits their responses:
- The revision task closes automatically — no action needed from you
- The questionnaire status returns to Completed
- All completed versions are stored within the PDF export
- You can view the updated responses from the questionnaire review page or the task
Frequently Asked Questions
Can I request a revision more than once? Yes. If the resubmitted answers still need corrections, you can send the questionnaire back for revision again using the same process.
Will the business see my original note from when the questionnaire was first sent (if it was sent on-demand/one time)? Yes. If you included a note when a one-time questionnaire was originally sent, the business will see that alongside your revision request note for full context.
Does the original questionnaire task stay in my Tasks table? Yes. Both the original task and the new revision task appear in the External Tasks table. The revision task links directly to the same questionnaire results.
What if I need to send an entirely new version instead? Use the Send New Version button on the questionnaire review page. This is required for Green Check Managed Templates and available for any questionnaire if you prefer to start fresh. The reason is because Green Check Managed Templates drive additional behavior so modifying them could lead to potential issues. The exception is Beneficial Ownership, which is a Green Check Managed Template that can be sent back for revision.
Is the revision captured in reporting or the audit trail? Yes. Both the revision request (who sent it and when) and the resubmission (who completed it and when) are recorded in the audit trail.
