Completing the Unverified Sales Questionnaire

This article explains why you may have received the unverified sales questionnaire and how to respond to it

If you’ve received an Unverified Sales Questionnaire in Green Check, it means your financial institution has flagged one or more sales in your POS data as unverified — these sales appear inconsistent with compliant sales activity.

This article explains why you received the questionnaire and how to complete it.

Why You Received This Questionnaire

Your financial institution uses Green Check to monitor sales for compliance.
If unverified sales are detected, this questionnaire is sent so you can:

  • Correct errors in your POS, or take another corrective action

  • Explain the data issues

What’s Included

When you open the questionnaire, you’ll see:

  • A downloadable CSV file — A snapshot of the unverified sales that need review.

  • Action options — Select what you did to address the discrepancies.

  • Required explanation field — Describe the steps you took or why no action was necessary. This field must be completed for every submission.

Steps to Complete the Questionnaire

  1. Go to your Inbox
    1. You'll find the Unverified Sales Questionnaire (Retail) to complete. The name of the questionnaire might also include the frequency.

    2. Open it to find instructions.

  1. Download and Review the CSV

    1. Click the download link in the questionnaire to view the unverified sales.

    2. Review each sale to determine if corrections are needed.

  2. Select the Action You Took

    1. Options include:

      1. Updated my POS source data 

      2. Identified and corrected SOPs

      3. Contacted my provider for support

      4. No action taken

      5. Other

    2. Choose the one that best matches your resolution steps.

  3. Provide an Explanation

    1. The text box is required for all options.

    2. Explain what you did (or why you didn’t take action) so your financial institution has a clear record.

  4. Submit Your Response

    1. Click Submit to send your answers back to your financial institution.

Note: If you selected Updated my POS source data, Green Check will automatically reprocess your sales data for the given time period. Fixing this source data and reprocessing should ensure that the sale is no longer 'unverified' and instead 'Green Checked'.  You will receive an alert when the re-ingestion is complete.

Tips for Completing the Questionnaire

  • Be specific: Include relevant details so the FI can quickly understand your actions.
  • Use the CSV as a reference: It represents sales exactly as they appeared when the questionnaire was sent.
  • Respond promptly: Delays in addressing unverified sales could affect your compliance status.

Once submitted, your financial institution will review your response. Note: You’ll only receive this questionnaire when there are unverified sales that need attention.
